Get a plastic policy. Plastic CHI policy: where to get a new health insurance card. You can get a plastic card

The issuance of a new sample of medical policies caused a slight stir among citizens. Despite the fact that electronic cards have been issued for several years, not everyone has yet decided to replace old paper documents with them. Employees of the Ministry of Health of the Russian Federation strongly recommend that citizens take up this issue, and in as soon as possible carry out the replacement. What is plastic compulsory medical insurance policy and what it gives, you will learn further.

Main advantages and disadvantages

There are obvious differences between an old-style document and a new plastic card. However, all innovations have their pros and cons. Let's consider them.

Advantages of an electronic card (EPOMS):

  1. Mobility and compactness. If we compare a plastic card with an A5 format document, then it clearly wins, since the paper carrier cannot even be folded in half, for the reason that the bend line falls directly on the barcode. In the case of EPOMS, this problem does not arise - it easily fits in a purse or business card holder.
  2. Strength and durability. An electronic card cannot be torn, which can easily happen with a paper carrier, and in order to break it, you need to make an effort. In addition, the plastic pole does not fade, and the data on it does not wear out over time - all this is typical of paper documents.
  3. The insured person does not incur any monetary costs, since the exchange takes place free of charge.
  4. The presence of a photograph and a chip with information is a guarantee that the document cannot be used by another person. To use this tool over the Internet, you will need a pin code.
  5. Together with EPOMS, you do not need to present a passport or other identity card - all information is contained on the chip.
  6. EPOMS is issued personally by the future owner, and is not tied to his place of work.
  7. Having a plastic card, you can make an appointment remotely - via the Internet or a terminal, and not waste time in queues.

Disadvantages of EPOMS:

  1. Not all branches of insurance organizations can produce this type of document, but it is only a matter of time.
  2. If some serious changes are coming, for example, a change of surname or place of residence, the owner of the EPOMS will be faced with the fact that he will have to change. It is not possible to overwrite data on the chip. There is only one way out: you need to get a new card.
  3. If you are planning a trip somewhere in the outback of the Russian Federation, it is better to take your passport with you in order to be able to properly apply for medical help. The problem is that not all clinics have the appropriate equipment to read data from the chip.

On a note! If the insured wishes to change insurance company, he can do this without changing the electronic card. It is enough to enter a personal PIN code, which is issued together with the EPOMS, and the insurance worker will independently update the information on the chip.

Appearance of plastic health insurance

In terms of size, a plastic CHI corresponds to a standard bank card. At one time, insurance was issued as part of a universal electronic card (UEC) - it was both an identity card and payment card, and a medical policy. However, they are currently discontinued. What does EPOMS look like today?

On the front side is:

  • individual insurance number in the form of a 16-digit code;
  • emblem of the CHI system;
  • coat of arms Russian Federation;
  • a microchip with data about the cardholder, about the insurance program and about the insurer;
  • the name of the insurance company and its contact details.

On the back you can see:

  • a digital photo of the insured and the serial number of the card under it;
  • Full name, signature, gender, date of birth of the owner;
  • card expiry date;
  • telephone number of the regional insurance fund;
  • icon with the abbreviation of the name of the document - POMS.

Who should apply

Citizens of the Russian Federation and foreign citizens temporarily staying in the country are entitled to receive medical insurance. Do you need to change your policy? You can use it until its expiration date. A document on a blank form of a new sample can be obtained at any time.

Today, medical insurance is issued once and for life, but there are cases when it is necessary to change old policy on new:

  • after the expiration of the old document;
  • when changing personal data - full name or registration;
  • in case of loss or damage to the document.

If the old paper policy is valid, according to Art. 51 paragraph 2 of the Federal Law "On Compulsory Medical Insurance in the Russian Federation" it has the same force as the new a plastic card. Thus, by presenting an old paper document issued before 2011, the insured has the right to receive medical care.

Where to get a plastic compulsory medical insurance policy

The procedure for replacing the old medical insurance on a new plastic card will not take much time. Get paper or plastic document can:

  • in the branches of insurance companies that are engaged in the production of electronic cards. It is possible that for this you will have to change the insurer, since not all organizations are engaged in issuing EPOMS;
  • at the nearest branch of the MFC. To do this, you need to bring a package of documents to the operator of the center, from there the information is transmitted to the insurer, and after production, he sends the card back to the MFC department;
  • on some online portals. You can order medical insurance on the official website of the mayor of Moscow and on the website of public services in St. Petersburg. On the public resource public services, the filing of applications for the issuance of EPOMS is temporarily suspended.

A complete list of organizations that issue new medical insurance can be seen on the website of the Federal Compulsory Health Insurance Fund.

Documents for registration

To apply for a plastic health insurance card with a chip, it is enough to have the following documents with you:

  • civil passport or birth certificate;
  • old policy (optional);
  • mandatory account number pension insurance(SNILS) - for persons over 14 years old;
  • replacement application.

Underage citizens come accompanied by a parent or legal representative. The latter must have a passport and a power of attorney. It is enough for the parent to bring their own civil passport with them.

When an appeal comes from a foreigner or a person without a permanent residence permit, you will need to attach the following documents:

  • residence permit (for foreign citizens);
  • certificate of temporary registration (in the absence of registration).

At the time of making plastic insurance, the insured person receives a temporary certificate of insurance, which gives him the right to seek medical care on the same conditions.

How to use the card

The electronic plastic card contains information about when and for what purpose the patient visited the clinic, when he was in the hospital, what services he used and how much medical care was provided to him. However, not all insurers know how to use such a policy and what it gives them.

In medical institutions, EPOMS should be used in the same way as paper. It is provided to a health worker who, using a special program, reads information from it and enters new data on the provision of medical services to it. In addition, with an electronic card, you can remotely make an appointment with a doctor - through the terminal or through the public services portal.

Along with the card, the insured person receives an envelope with codes. A pin code and a pack code are needed in order to enter some information on the chip. For example, when changing the insurance company, you can simply enter new data on the card without changing it.

Replacing a paper policy with a plastic CHI policy

You can get a new CHI policy within thirty working days, but, as a rule, the card is ready within a couple of weeks after applying.

The main stages of replacing a paper CHI policy with a plastic one:

  1. Contacting your insurance company or choosing a new organization.
  2. Visiting an insurer or an MFC with a package of the above documents.
  3. Fill out an application for a replacement.
  4. Obtaining a temporary certificate of insurance.
  5. Making an appointment for a new insurance policy.
  6. Obtaining a CHI policy in the form of a plastic card at the appointed time.

Conclusion

A plastic card with a chip is a new type of compulsory insurance policy. health insurance. Thanks to this tool, the medical staff can at any time find out when the insured visited the clinic and what services were provided to him.

The card is also very convenient for its owner: it is compact, and therefore always with him, you can use it to make an appointment with a doctor without standing in queues, besides, she has high degree protection - it will not work to use someone else's document.

Health insurance plays a very important role in the life of any person. If we compare it with other types of insurance, we see that not every person concludes them. Health insurance, which is mandatory, is necessary for every adult and child.

Its presence guarantees that in case of illness or accident, timely and qualified medical care will be provided, which will be paid for by the state. In 2014, a law was passed that provides for the replacement of previously used insurance with new documents. The use of new documents can sometimes raise certain questions. Let's talk about them in more detail.

Dear reader! Our articles talk about typical solutions legal issues but each case is unique.

What you need to know about the new policy

It has been operating since the beginning of 2014. First of all, we note that it is necessary to replace the old insurance with a new one, but even without such a replacement, its use is permissible. When contacted, she guarantees the provision of the necessary medical care. The legislation did not establish a deadline for the exchange, after which the use of old-style documents is impossible. However, despite this, the new option has some very important advantages.

One of them is that it is the same for all regions of the Russian Federation. What exactly does this mean?

Previously, the compulsory medical insurance policy was valid only where it was issued and had no legal force in the rest of Russia. Now this situation has completely changed. Having received it in one region, you can use it throughout the state.

Another important advantage is that new insurance can be issued not only in paper form, but also in the form of a plastic card or in the form of additional service, which is included in the Universal Electronic Card (UEC).

MHI policy on paper:

The question may arise as to how a medical institution can read data from a plastic card or from a UEC? After all, not every clinic may have the equipment suitable for this. In order to answer this question, let's show how the new document looks in each of these cases.

If this is a plastic card, then in size it exactly corresponds to a regular bank plastic card. There is a front and back side.

The front side shows the name of the insurance company that issued it, a unique number and a computer chip that contains all the necessary information. The reverse side contains personal data, including a photo, last name, first name, patronymic, signature sample and some other data.

This is what the front looks like:

Now let's look at the reverse side:

Upon receipt of the UEC card, it is also easy to read the necessary data. They are on the back of the card.

As we can see, all the necessary data (including the insurance number) can be easily read without resorting to special equipment.

This is what the front side of the UEC card looks like:

And this is the reverse side:

If you received a new policy in paper form, then a unique insurance certificate number will be indicated at the bottom of it on the front side.

The figure on the left shows the front of the document, on the right is its reverse side. The number, which in the right figure is indicated by sixteen zeros, in the real document contains the number of the insurance policy.

Expiry date of the new document

Now there are policies that were issued at different times. Of course, it is more convenient and practical to use a new type of document, because it is valid throughout the country, unlike those that were issued before. However, at the moment, they are all valid, despite the fact that the expiration date that was once indicated there has already passed. The latter circumstance cannot be the reason for the denial of medical care.

As for the new policy, it has an unlimited validity period.

However, time limits still exist in some cases. They refer to those people who are temporarily staying in our state and the period of such a stay has expired. In this case, we can talk about those who received only a temporary residence permit in Russia (which has expired) or those who have refugee status, which has already expired.

When to replace it

Although in the new insurance policy CHI does not have a date after which it expires, yet there are certain circumstances as a result of which it is necessary to without fail replace this document:

  • When changing your passport for any reason. If it has become unusable, lost, due to a change of surname and for other similar reasons.
  • The old policy has expired. As mentioned above, this does not deprive the right to receive medical care, however, in practice, it can create some difficulties in processing the relevant documents.
  • If the old document has become dilapidated and important information cannot be read on it.
  • If the old insurance is lost.
  • If personal data has changed. This may be, for example, a change of surname, address of residence or other changes of this kind.

How to get a new type of policy

To obtain it, you need to contact the insurance company with a document that can verify the identity of the applicant. If the execution of this document may take some time, then the company issues a temporary insurance policy.

Its duration is limited and usually does not exceed one month. If registration does not end in a month, then temporary insurance is extended.

Documents required to obtain a policy

  • Upon receipt, an identity document must be presented. Usually we are talking about a passport. In the event that the policy is issued for a child, a birth certificate must be presented. It is also possible that the registration is made to a third party. In this case, you must have the appropriate power of attorney.
  • An existing document. Presenting it is desirable, but not mandatory.
  • An insurance certificate that relates to compulsory pension insurance. This document is desirable, but not required.

Who needs to get a new document first

Here is a list of them:

  • Newborns.
  • Previously did not have a CHI policy.
  • lost this document.
  • Name changers or name.
  • If the existing policy contains false information.
  • If there has been a change of residence, and in the new place there is no branch of the insurance company that previously issued it.

How to choose the right health insurance company

The insurance company that issues this policy does not directly affect the quality of medical services received. But she can control how they are delivered. The issued document is the same throughout the country. Therefore, they usually choose the company that they have previously contacted and were satisfied with or, if there is none, the one that is more conveniently located.

But let's not forget about the important circumstance that in case of violation of the patient's rights when receiving medical care, it is possible to contact the insurance company that issued the insurance certificate. Then it is likely that such treatment will allow the patient to solve his medical problems. The possibility of such treatment is important to consider when choosing the insurance medical company which makes sense to refer to.

How to get a CHI policy for a child

When receiving it for a child, you need to present a birth certificate as an identity document. The rest of the procedure for obtaining a policy is standard.

What medical assistance can be obtained under the policy

Each of us can get sick or injured. In this case, the state provides for the possibility of free medical care upon presentation of this document. The volume of such assistance, as well as the list of its types, is determined by the current state programs.

They are of two main types:

  1. Programs that operate throughout the country.
  2. Those programs that operate in certain regions.

In the latter case, services are provided in accordance with the region in which the insurance policy was issued.

We are talking about providing such types of assistance as:

  • primary;
  • specialized;
  • ambulance;
  • palliative care.

plastic card

It has the same legal force as a policy in paper form and is issued simultaneously with a paper document. In appearance, it resembles the usual bank card. The difference is that the necessary information is indicated here, not only in in electronic format but also visually.

There is a photo, signature sample, certificate number and other necessary personal data. In addition, it is not uncommon for a paper document to be folded several times for ease of storage. This can lead to its abrasion and the inability to read the necessary information. The plastic version is devoid of such a drawback. It is convenient and practical both when using it and when storing it.

Conclusion

Although the replacement of the old policy is not strictly mandatory, nevertheless, obtaining a new document will allow you to feel more confident when receiving medical care.

In SOGAZ-Med, to serve people with handicapped, including disabled people, individual services are organized for filling out applications, issuing temporary certificates and issuing compulsory medical insurance policies at home, in accordance with clause 70 of the Rules for Compulsory Medical Insurance, approved by Order of the Ministry of Health of Russia dated February 28, 2019 No. 108n.

The procedure for replacing the OMS policy with a new model policy started several years ago. Get a policy oms new a sample will help a personal appeal to a company specializing in health insurance, or to the MFC. Alternatively, it can be ordered through a trustee. Be sure to indicate the preferred form of the received document, which is available for:

  • people with Russian citizenship;
  • people who are citizens of other states but reside in Russia on a permanent or temporary basis;
  • refugees;
  • persons without citizenship.

It is required to provide its owner with medical care without payment throughout the territory of the Russian Federation, giving the right to choose a medical institution, calling on polyclinics, hospitals, medical centers to work efficiently and efficiently.

What does he look like?

A new policy can be obtained at:

  • in the usual form - a paper document with data placed on both sides;
  • compact form of a plastic card;
  • as part of a universal electronic card (prospective option).

What does a new OMS policy look like in paper form? The format of the new document is A5, it is not recommended to bend it, and it is strictly forbidden to laminate it. The only thing that can be done is photocopies on both sides, which will help extend the life of the original. On the front side there is information about the owner (name, date, month and year of birth), date of receipt, a unique barcode. On the reverse side, information about the place of residence, the insurance company and its coordinates is printed.

The electronic form is similar to a bank card. All data about who issued it is stored on the CHIP, for this reason, owners of plastic insurance should remember the name of their insurance company in order to provide information at hospitals and clinics. There is also a unique number here. On the reverse side there is a photograph and signature of the owner, the date until which the card is valid. The answer to the question of where to get a new type of OMS plastic policy requires additional preparation related to calling organizations whose activities are related to health insurance. The reason is that not everyone has the technical ability to issue a plastic version.

You can check the authenticity of the paper or plastic version of the policy issued to you on the official resource by entering a unique number in a special search bar. You will need to turn the document face to face, where the OMS policy number of the new sample is printed, regardless of whether it is made on paper or plastic.

List of documents

The documents for obtaining a new OMS policy are, in principle, the same for everyone:

  • a correctly executed application to an organization whose specialization is medical insurance;
  • a certified copy of a document by which you can unambiguously verify your identity;
  • certified copy of SNILS (if available).
  • a certified copy of the document confirming the status of "refugee" (certificate of consideration of the application on this issue), residence permit, identity document of a stateless person, where there is a mark confirming the legality of residence in the territory of the Russian Federation for foreign citizens, refugees and stateless people respectively.

By submitting an application for a medical insurance document of a single sample, the applicant receives a certificate in his hands that replaces the policy and gives the right to receive medical assistance until the registration procedure is completed. This certificate can be used no longer than thirty working days, after which the original medical insurance document in the form chosen by the applicant must be received in hand.

Regardless of whether you are a citizen of the Russian Federation or not, getting a new OMS policy is quite simple and fast.

Do I need to renew or order a reissue

First of all, it should be emphasized that all medical insurance documents received on hand, dated January 1, 2011 and earlier, are required to be accepted by any medical institution. Change or not old version is the business of its owner.

It is worth paying attention to the fact that having received a new document in your hands, you will not find an expiration date on it. Therefore, to the question: “Do I need to renew the OMS policy of a new sample?” the answer will be negative with some caveats:

  • there are no restrictions on the validity of this document for citizens of the Russian Federation;
  • for people who have received the status of "refugee", the policy will be valid until the person leaves the country;
  • for people who have a temporary residence permit on the territory of the Russian Federation, he will help to receive medical care free of charge as long as the permit is valid.

Advantages

A single OMS policy of a new sample is valid on the territory of all subjects of the Russian Federation, regardless of the residence permit or registration of the owner. For this reason, on any trip within Russia, you must take it with you. A single OMS policy of a new sample gives the right to choose not only a doctor who provides medical services, but also a medical institution.

Nuances of obtaining a new sample document

As in any case initiated by the Russian authorities, upon receipt single policy guaranteeing medical care, there are a number of nuances for which you need to prepare.

You can change the insurance company once during the calendar year and strictly until November 1. The exception is the owner's new registration address.

Insurance is carried out by one organization at a time. Accordingly, a person can be the owner of one policy.

If the plastic format was lost or damaged, then only its paper counterpart can be issued to replace it.

The plans of the government of the country over time to move massively to the universal electronic map, which will replace policies (not only old, but also new), acting as an identity card, certificate of pension insurance, etc., opening up access to many state and municipal services in any region of the Russian Federation without payment.

Now any polyclinic providing emergency care or long-term treatment, as well as local doctors who come to the house for the purpose of examination, will definitely be asked to present a passport and a medical policy. This is a document issued by an insurance company for receiving medical services free of charge.

To make it convenient, comfortable and practical, it was decided to improve the existing document. Further in the article, we will learn in more detail what a plastic card of a new type of compulsory medical insurance policy is, the place of its receipt and who has the right to use such an innovation.

The issue of compulsory health insurance has always been relevant and in order to receive free assistance, it was necessary to issue an insurance policy. Control over this was previously assigned to employers, student organizations, school and preschool institutions.

In 2011, the introduction system compulsory insurance citizens began to transform, and in order to unite and regulate the process of issuing forms, they decided to issue a single perpetual compulsory medical insurance policy on blue paper.

It is issued on a sheet equal to A5 format and is valid throughout Russia, regardless of place of residence and registration. But, the operation of paper insurance, unprotected from mechanical damage, is impractical.

But since 2015, medicine has been able to change the approach and found a solution to this problem. Now policyholders have begun to issue an electronic compulsory medical insurance policy. It looks like a plastic card equipped with a chip, which contains all the information about its owner.

The essence of such a decision is simple, to everyone who, on mandatory terms, insurance companies issued policies that have a limited validity period, at the end of it, they have the right to apply there in order to receive a new insurance with an electronic carrier.

What does the CHI policy of the new sample look like?

The new compulsory medical insurance policy has the format of an ordinary plastic card, similar to a bank card, which we use daily to manage our own funds.

Its single sample is decorated with the colors of the Russian tricolor, and on the front side there is the following information:

  1. Image of the coat of arms of the Russian Federation;
  2. Insurance system logo;
  3. Chip and policy number;
  4. The name of the SC.

On the back side, all personal information is indicated. Here is placed a photo, full name, date of birth and gender. You will be asked to sign. Looking at this form, you will definitely notice a hologram confirming the authenticity of the insurance.

There is also a phone number of the fund that issued the document. The term of the MHI policy is 5 years. That is, after this time, you will have another card exchange.

Difference from paper

New medicine with such insurance allows you to receive free treatment, determining the list of services available to the consumer. This means that by contacting polyclinics, medical institutions, hospitals in our country, we have the right to service, assistance, as well as the provision of certain medicines.

Not all regions are ready to accept modern system and take advantage of it. So, the lack of complete data about your company can seriously complicate treatment and provoke additional difficulties. But, the policy of the new sample has a number of differences from the old one.

  1. The size. The small format allows you to get a plastic card and store it in your wallet or passport cover, which is more convenient for the owner. When the paper version was not allowed to be laminated and folded across, and this required special handling.
  2. Material. Electronic policy OMC has a plastic base and is practically not subject to mechanical and physical influences. Paper insurance is easy to wrinkle.
  3. Data security. The new CHI program provides for protection against the use of services by other persons, since now the citizen’s personal information is printed on the card, and a photo is also posted. Such insignia and chips make it possible to create a unified information base of policyholders.

Where to get a plastic OMS card

The compulsory medical insurance system covers the entire territory of the Russian Federation, therefore, all citizens can protect their lives. This means that the able-bodied population, pensioners, students and schoolchildren, and even just born babies and the unemployed can count on protecting their health.

There are several ways to get a card:

  1. Protecting Organization (SC). Bringing old-style insurance, a paper form, without spending time and effort, you replace it with plastic. The operator will have access to your series and number of the CHI policy on a single basis. There he will see your personal data.
  2. MFC. A multifunctional system for the provision of state and municipal services will accept your documents. You need to register an application and within 14 days, an SMS will be sent to the specified number about the service performed. https://www.mos.ru/pgu/ru/md/

  4. State Services Portal. You can apply for an OMS policy online. To do this, you must fill out an electronic form, enter all necessary information and choose a pick-up point that is close to you geographically. https://gosuslugi.help/polis-oms.html

Insurance companies

CHI allows you to choose an insurance company yourself. So, if you are not satisfied with the quality of the services provided, then the replacement of the insurance agent is possible once a year.

There are many companies working in the field of insurance of citizens, here are some of them:

  1. VTB Medical Insurance
  2. "Spassky Gate -M";
  3. "Ingosstrakh-M";
  4. "RGS Medicine";
  5. "Uralsib";
  6. "Consent -M", etc.

This list can be endless, but they are all suitable for protection. The rating of their popularity varies depending on the requests of insured citizens, but these are huge companies and it will be more convenient to get a new policy here.

Registration of a new electronic policy

The CHI policy of the new sample, as you already understood, looks like a plastic card. But not important appearance insurance, and how it is issued and a list of documents required in the future.

You will be asked to present:

  1. Passport of a citizen of the Russian Federation;
  2. An application filled out on the form of the IC dealing with protection;
  3. When applying for a new type of insurance for a child under the age of 14, you need metrics (birth certificate).

In 2018, you can protect yourself and get a card within 30 days. During this time, the policyholder is given a temporary certificate that replaces the card, but it also indicates the number and series, as well as personal data.

So, if in the allotted time to prepare the card you have to get help or treatment, stay in a hospital, buy drugs, consult a doctor, then feel free to use temporary insurance.

During the application, write down the phone number to contact the operator. The CHI policy is usually changed quickly, no more than 11 days. But try to control the whole process yourself and do not wait for someone to call you, for these purposes, take the operator's number.

Pros and cons of new policy

We told how to replace the CHI by last name with a new plastic format, but how to use such a document? The opportunities that a plastic card provides have not yet been fully implemented in many regions, and therefore there is a possibility that you will not receive proper treatment and required services for free. The table shows some of the pros and cons of both types of protection.

Type of insurance Advantages Flaws
old pattern The right to make changes to the form Inconvenience to use
Habitual in use for medical professionals Opportunity to receive services from third parties
New pattern Mobility, durability, ease of use Lack of device in honey. policy reading agency
High level of protection When changing data, it is obligatory to obtain a new policy
No outside use Not every insurer is able to issue a new policy

Both insurances have the same legal effect and are not subject to any differences in medical care.

Moscow and region

CHI points allow you to get a policy. And this is allowed both during the initial treatment and for other reasons.:

  • When changing personal data. Marriage, another place of residence, name or patronymic necessarily contributes to a new (repeated) entry of information;
  • Wrongly entered information. When applying for insurance, the operator introduced an inaccuracy that requires it to be replaced;
  • When it is lost. There are many reasons for this randomness. But, if a loss occurs, write a request for restoration;
  • Obscene state. If you understand that the document is damaged and indecent, it must be replaced.

The places where you can get a CHI policy in Moscow are mainly related to the addresses of insurance companies.

In the Moscow region, it is easier to issue and find out if the compulsory medical insurance policy is ready at the MFC branches, at addresses such as:

  • Schelkovo, st. Svirskaya, 2 A;
  • Orekhovo-Zuevo, st. Lenina, 96 A;
  • Lyubertsy, Oktyabrsky Avenue, 190;
  • Balashikha, st. Soviet, 4.

Where is the number and series on the policy

For an insurance organization where cards are issued, there is no difference between citizens and their status. And this means that an unemployed or retired person, a student at school or an ordinary worker has the right to use the services under the CHI policy on an equal footing. Therefore, the appearance of the map is the same. Only insurance issued for a child under 14 years of age is eligible for an exception.

In the case of a child card, the photo of the child is not placed. On the front side of the card are the series and number of the CHI policy in the form of a 16-digit code. Due to the chip on which the information is stored, the devices quickly recognize the data and determine the owner of the insurance. So, having lost a card after replacing it from paper to plastic, it is possible to restore all data and the card itself through the number.

What is the validity period of the new CHI policy

Early replacement is made only in case of change of information or in case of loss. How to restore the CHI policy in this case? Report the incident to your insurance organization, take your passport and write an application. There you will quickly be helped to restore the document.

The new format of insurance has limitations, which means that its usefulness will be extended for no more than 5 years. You can determine the expiration date of your card by looking at its back. When the specified period begins to come to an end, a replacement is required.

The replacement process will not take much time, since the list of documents will not change. All you need is to write an application and wait for a new card within 1 month.

When paying compulsory medical insurance contributions to the employer, we have the right to independently choose the institution in which we are ready to receive services, the insurer, and even between types of insurance. Each one is capable of determining which one is convenient, and only the order of further actions will depend on your decision.
